    • Job Requirements
    • Must have a valid state nursing license
    • BLS certification required for all direct patient care personnel
    • ACLS certification required
    • License/Certificate Verification must be dated within 30 days of start
    • Fit Test
    • Trinity 9 Panel Drug Screen must be completed within 30 days of start date
    • Proof of vaccine or declination for COVID-19
    • Upload proof of various vaccinations
    • Complete Physical/Health Statement
    • Complete all other required professional and personal health history certifications
      • Schedule Information

        • Additional Details
        • All engaged contractors must have page 27 signed form on file in Trinity VMS.
        • Must have Trinity specific credentialing forms completed
        • Federal, Nationwide, County Criminal background checks are required dated within 30 days of start
