Job Description:
A company is looking for a Patient Navigator II to guide
patients through the healthcare system and ensure access to
appropriate services.
Key Responsibilities
Provide advocacy and case management services for patients,
assessing their emotional and social needs
Assist patients in accessing healthcare support services and
maintaining medical insurance coverage
Document patient encounters and comply with regulatory and contract
requirements for data collection
Required Qualifications, Training, and Education
Associate's Degree required; degree in social work, psychology, or
related field preferred
Bachelor's Degree preferred
No previous healthcare experience required
Familiarity with community, social, and health resources is
preferred
Bilingual or multi-lingual skills appropriate to the patient
population served are a plus
