        • American Heart Association certification is required for all direct patient care personnel. Attach scanned copy of certification (with expiration date) to candidate's record in TrinityVMS. Must include an image of the front and back of the card.
        • Proof of vaccine, declination or exemption (with agency letterhead). Declination not accepted for CA or IL.
